All products returned to Midwest Micro-Tek must be assigned a RMA number. To obtain this number, please call Midwest Micro-Tek and request an RMA form, which will be faxed to you with an assigned RMA number. Please provide the following information on the form and return it along with the product to be serviced.

  1. A description of the product(s) to be returned including, quantity and serial number(s).
  2. A general description of the problem and the steps you have taken to resolve the problem.
  3. Invoice number and date of purchase (if available), and original purchase order number.
  4. Name, address, telephone and FAX number of the person making the request.

Failure to provide a general description of the problem will result in an evaluation charge of $65 even if the board is under warranty. The evaluation charge will also apply to any product that Midwest Micro-Tek finds to be operational and non-defective. Do not debit Midwest Micro-Tek for any repairs, Midwest Micro-Tek does not authorize debits.

After receiving the RMA number, please return the product promptly. Make sure that the RMA form is included in the shipment and the RMA number is visible on the outside of the package. All returned materials must be shipped prepaid and insured at the customer’s expense. Warranted products will be returned to domestic customers via UPS ground at Midwest Micro-Tek’s expense. If the customer specifies an alternative method, additional charges will apply. Returns to international customers are at the customer’s expense.

Returned products must be enclosed in a sealed anti-static bag to protect it from damage caused by static electricity. Packing material must separate each product returned and placed as a cushion between the products and the sides and top of the shipping container. Midwest Micro-Tek is not responsible for any damage to the product due to inadequate packaging or static electricity.
